Terms and Conditions Therapeutic Agreement - Informed consent

The present therapeutic agreement is between you (the Client) and Laura Hawthorn (the Therapist), the session refers to a booked time slot previously agreed.

By commencing therapy, the Client (you) agrees to the following terms and conditions:

All new Clients are required to read and accept the terms and conditions of this agreement. The Client is required to respond in writing (via email) if s/he does not agree to the terms and conditions of this agreement.

Punctuality, any Client more than 15 minutes late for a session and the session will be considered cancelled.

Any session cancelled or rescheduled by the Client with less than 24hrs notice will be charged in full to the Client. 

These T&C's apply to face to face and online sessions

The client can be contacted from the Therapist relevant to the therapy by, phone, email, text at any time.

The Therapist requires the client to be truthful and open in the sessions and work in the best interests of the therapy being provided.

It is not the responsibility of the Therapist to achieve the desired outcome for the Client, especially in relation to smoking cessation. The desired outcome as an achievement of the goal(s) cannot be guaranteed or promised to the client by the Therapist. It would be unethical to provide a guarantee for a Patient’s treatment of therapy.

Standard sessions are £60 per session (50 minutes), unless otherwise agreed.

All Sessions must be paid for by bank transfer no later than 24hrs after the session.  Bank account details for payment will be sent by email to Clients in advance

The Therapist has the right to cancel or stop a session at any time if they consider the Client to be a risk to themselves or anyone else.  The appropriate services will also be alerted in all cases of violence or personal threats.

Any Audio recordings or documents shared with the client is to be used only by the Client and still considered to be the Therapists intellectual property.

The Client is at no time allowed to record (vidoe or audio) the sessions unless the Therapist has issued written consent.
